I don't think it looks like they are shaking hands. The title of this artwork is "telling" by Steve Walker, an artist whose work is mainly on gay related art.
For me when I first saw the pic, I felt sadness and also a lot of love between the two individuals who were holding hands. Perhaps one was trying to tell the other that he is dying of AIDS or perhaps he has decided to commit to a relationship with the other guy? Bery 'Brokeback' I know but that was how I felt and I feel the picture captured the feelings well.
