Post Call and non the worst off

Had a relatively good sleep last night while on call. Slept at 1 am after reviewing the last patient. He suddenly developed typical left sided chest pain which lasted a few minutes. By the time I got to him, he was well again but his ECG showed significant changes. I treated him as unstable angina till further review.

Was paged at 3 am by the MO downstairs regarding 2 patients. One was another geriatric patient who was promptly dumped in the A&E by the family and hence, again, we have no idea what she has! From her Xray she appear to have pneumonia or maybe even an abscess. Doesn't it make your blood boil that people can be so heartless? I mean how can you just leave your loved ones (assuming they are loved) in the ER and scoot off?!

The other was a referral from the psychiatrist. The patient was a 31 year old guy who was depressed and wanted to jump off a building to kill himself. The referral was for an incidental finding of a high white cell count and a slightly elevated paracetamol level. We didn't take over the patient as my MO could not find a source of infection nor any evidence of infection (note: a high white cell count does not necessary always mean an infection) and anyone who has just taken even just 2 tablets of panadol would have an elevated PCM level.

Thereafter it was sweet sleep till 7 am this morning. I don't feel very tired, maybe later, after lunch perhaps. I have 5 medical students to teach at 10 am. Btw, they didn't show up last night for the rounds. I did ask them to choose between watching Desperate Housewives or the rounds. Obviously they love Bree, Gab, Lynette, Susan and all the hunky guys in the show more. No-lah I think they were probably busy clerking the 4 patients I assigned to them yesterday.

It's the middle of the week! Am looking forward to the end of the week. Planning to take my little nest for a weekend getaway somewhere in the mountains.
